Job Description

Job Title: Community Link Worker (Social Prescribing)
Location: Derby City North (across GP practices and community settings)
Hours: 29.5 hours per week (flexible across Monday to Friday)
Job Type: Permanent

Priority Recruitment are excited to present this rewarding opportunity for a Community Link Worker (Social Prescribing) to support individuals across Derby City North to improve their health and wellbeing through personalised, non-medical support.

This role focuses on building strong, trusting relationships with people, helping them identify what matters most to them, and connecting them with community-based services, activities, and support networks that address the wider determinants of health.

Company and Role:

  • Work across GP practices within the Derby City North Primary Care Network
  • Deliver person-centred, asset-based social prescribing support
  • Support individuals experiencing loneliness, isolation, long-term conditions, mental health challenges, and complex social needs
  • Reduce health inequalities by addressing issues such as debt, housing, unemployment, and physical inactivity
  • Build strong links with voluntary, community, and statutory organisations

Key Responsibilities:

  • Accept referrals from GP practices and other professionals
  • Undertake one-to-one sessions, including home visits and community-based meetings
  • Co-produce personalised wellbeing action plans focused on prevention, independence, and resilience
  • Support individuals to increase social connectedness and access community activities
  • Identify risks and escalate concerns appropriately when needs fall outside the scope of the role
  • Work collaboratively with community groups and voluntary organisations to facilitate referrals
  • Capture data to monitor outcomes and demonstrate the impact of social prescribing
  • Promote social prescribing across GP practices and partner agencies

Successful Applicant:

  • Strong ability to listen, empathise, and provide non-judgemental, person-centred support
  • Commitment to reducing health inequalities and supporting diverse communities
  • Excellent verbal and written communication skills
  • Ability to assess, manage, and respond to risk appropriately
  • Experience supporting individuals, families, or carers in a related role (paid or unpaid)
  • Experience of case management and multi-agency working
  • NVQ Level 3 (or equivalent) qualification, or working towards
  • Knowledge of personalised care and asset-based approaches
  • Confident using IT systems for recording data and reporting outcomes
  • Willingness to work flexibly, including occasional Saturday mornings if required
