£25,000 – £30,000 | Central Manchester (Hybrid)
As long as you can be in Manchester 2 days a week, we are open to any home location in the North West

About the role

As a Care Coordinator, you’ll be responsible for planning, managing and improving support for people with complex physical and/or mental health needs. You’ll work closely with support workers, families, healthcare professionals and therapists to make sure support is consistent, safe and truly person-centred.

You’ll need to be proactive, highly organised and passionate about delivering the best possible outcomes. This role is community-based, so a full driving licence and access to a car are essential.

You’ll also be part of the on-call rota, helping to maintain continuity and responsiveness across our care packages.

What you’ll be doing:

  • Create and review personalised support plans and risk assessments.

  • Visit service users regularly to ensure high-quality care is being delivered.

  • Liaise with families, therapists and healthcare professionals to coordinate care.

  • Supervise, support and develop a team of Support Workers.

  • Help onboard and match new staff to service users based on skills and compatibility.

  • Ensure accurate record keeping and maintain compliance with CQC standards.

  • Troubleshoot issues quickly and professionally, keeping everyone informed.

  • Uphold the values and reputation of the organisation in everything you do.

What we’re looking for:

  • Experience in a similar care coordination or service delivery role (essential).

  • Full UK driving licence and own vehicle (essential).
  • Experience supporting people with complex or long-term needs.

  • Great communication skills and the ability to build strong working relationships.

  • Highly organised, with excellent attention to detail and follow-through.

  • Ability to write clear support plans, risk assessments and professional correspondence.

  • Confident using digital systems and Microsoft Office.

  • Problem-solving mindset and a calm, positive approach.

Bonus points for:

  • Experience working with CQC-regulated services.

  • Knowledge of acquired brain injury, autism, or mental health.

  • Experience using digital care management systems.

Contract type: Permanent, full-time
Hours: Monday–Friday
Location: Hybrid – Central Manchester office + community visits
On-call: Required as part of a rota
